By submitting a Request for Proposal (RFP) form(s) I am authorizing EmployHR.com to provide this information to its partners as required in order to obtain obtain pricing proposals on my behalf for Professional Employer Organization (PEO) services. 

I further understand that:

a.) Under no circumstances will my organization be liable for fees to EmployHR.com for RFP services.
b.) EmployHR.com will assist my Company in identifying a PEO(s) that can provide my Company with PEO services but that EmployHR.com is not a PEO nor does it provide PEO services.
c.) That EmployHR.com makes no guarantees regarding the capabilities or solvency of any PEO and that I will hold EmployHR.com harmless for any misrepresentations or omissions made by any PEO or PEO Agent/Representative.
c.) Any information I provide is true and accurate to the best of my knowledge.
d.) EmployHR.com does not guarantee that I will receive any PEO proposals or quotations. EmployHR.com is not acting in any legal capacity as an agent or representative of any PEO and EmployHR.com has no liability for any damages that may result from my Company's relationship or lack thereof with any PEO.
e.) EmployHR.com will not be responsible for any damages regarding obtaining or maintaining any required insurance.
f.) EmplyHR.com is owned and operated by Everett HR, LLC. Everett HR, LLC is not an insurance agency and does not provide tax advice.
